VGA PROXY FORM

Our Association requires a 51% vote of our total paying members to achieve a quorum so that the Association can vote on matters important to us at our regular business meetings.

Members have three options:

• Do not provide a proxy.

• Name the MAJORITY VOTE to serve as your proxy for this meeting or every meeting.

• Name a specific member to serve as your proxy for this meeting or every meeting.

If you decide to submit a WRITTEN proxy, you have two options:

Complete and submit the form below, or print out a proxy form and deliver it to the Information Desk at the VNMP before the scheduled meeting.  A written proxy should be sealed in an envelop with your name on the outside.
